After reading through this story I have wondered how there will be any reconciliation with the overwhelming sorrow John must be going through. Any human would not want to go on living after everything Lily has put him through. There is a lot of hints as to him not enjoying being used in this manner by his sister. I think it would be interesting to flesh out a scenario where Lily has to overcome his complete broken will to live after this day of torture if this is what he  has to look forward to as she made clear will be for the rest of his life. I feel there should have been more introspection on his part with his mental health after her spending over $1,700 on all those toys as well as the torture of being branded in such a painful way, the overwhelming torture he went through in being subjected to staying in Lily's mouth during her eating and drinking, after his only recourse is to lash out she subjects him to hours of being under her sandal as she walks through town.  and then the unrelenting torcher of being used as a living dildo being completely engulfed in her vagina and anus. This would break any human to the point of not wanting to live any longer. After she made him give her ideas with the promise of being able to rest and him giving into her without her holding up to her promise but rather placing him in her panties and then farting on him. The dehumanizing effect it would have on a person would create more of a devastating break from reality. I would think he would just mindlessly start to beg for her to end him. This would make her need to take stock in what she is doing and how she wants to move forward with him in a manner that would ease him into submission rather that this abrupt method. This is a very engaging story and has my interest peaked to say the least and am looking forward to reading more when you post. I truly hope you take my opinion for what it is "My opinion"
