I like TomSpeedy idea Tom should be conscious when being sewed.
The natural submissive (which Tom obviously is) needs to feel he is helplessly under the control of a dominant and has zero power or influence. One way how dominant can achieve it is to let submissive undergo things he is truly feared, afraid or does not like. By this it is reinforced he has no power or influence and he is really under the control of a (sadistic) dominant.
That is why it would be so interesting to see Tom nervous but turned on enjoying he is helpless and fully under the control of his teacher Miss Harwood. Another extraordinary chapter!
I like Katie's did not start to feel sorry for Tom after pulling him out. She tried to awake him just because she wanted to continue in her pleasuring - which indicates how deliciously cruel she can be. I would almost say she pursed her lips not because of Tom condition but because she got a bit frustrated she had to interrupt her enjoyment. (I am sure she will tie him on securely next time ;-))
