Wow, i love this story.
The Idea is great, please please continue it.
In my opinion, the mother and sister could be a lttle bit more cruel. Perhaps they could force him to lick their toes or breasts after he failed a task for them. They shout treat him more like a slave or a pet. They could aqueeze them betwenn their boobs for their personal arousal. They could force him to lick their nipples.
Perhaps the mother or sister could start breastfeeding him, when the Food is too big form him.
Whe you need other Characters, you could put him at the mercy to his cousin or his grandmother (you metioned her before), who are even more cruel and demanding towards him. The grandmother could force him to massage her toes all day and lock him in her shoes.
Anyways, i love the story and please apologize my bad english, its not my mother language.
When you bring Sarah back, you could show us how many slaves or pets she has now and how she treats them. Perhaps allisson can come back a week later and look what sarah does.

I think a direction to go in could be mom finds out about John being given to Sarah for the day and decides she cant let John out of her sight. He now has to go everywhere with her. And not all of her clothes have pockets. After some after work drinks she starts to think about the fun to be had with her little man, while remaining gentle and caring but meeting her needs also.
