With Karen's delusional narcissism and Amanda's selfish manipulation, this really didn't have to be unaware. I bet Karen would take it as a personal triumph if she "convinced" Sam to straddle Amanda's cock and let her blow them both at the same time.

I don't think Karen's necessarily a narcissist. Delusional, sure. And desperate. Desperate to be delusional. But she still loves her son more than anything--except getting high.
The unaware nature of that scene was part of the comission, but I think there's pretty clear evidence that Amanda looks past it in order to continue getting laid. I tried to make the future of the nature of the three's relationship fairly clear, though it won't be gone into detail as the story reached its end here (sad, I know). I tried to make the metaphor that Amanda is willing to fuck Sam over by catering to her own interests above all else clear, to give that final sex scene some heavy symbolism in what it means for the future.
